Seeing is Believing - Co-production Case Studies from Wales

Gwynedd Council and Betsi Cadwaladr University Health Board (BCUHB) have established an integrated health and social care team for older people – the Ffordd Gwynedd Health and Care team. This jointly-managed, integrated team includes social workers, district nurses, occupational therapists, enablement officers and third-sector professionals. Additional expertise is brought in as required from physiotherapists, community psychiatric nurses and third-sector organisations. The team identify and remove any blockages in the system to ensure a timely and effective service that focuses on what matters to the people we support. Our aim is to ‘help me to live my life as I want to live it’. The initiative is in its early stages (operational in one locality of Gwynedd at present) but we are already seeing the benefits of this integrated, person-centred approach.
